This is a failed attempt at Subterranean Animism's Extra Stage using Reimu-A. This is my best run at it so far.
I've never been able to complete an Extra Stage for any of the main Touhou Project games. I never really tried too hard, though. I knew that completing an Extra Stage would require lots of practice and dying, and I was content enough with being able to just unlock it. However, out of all the games I've played so far, I feel that this is one that I actually have a chance of beating if I practice some more.

The actual stage before the boss isn't too difficult. I make some small mistakes, but I'm able to reach full power without dying and make it to the boss. I know that I let a lot of points fall because I'm not grazing enough, but getting a high score isn't my goal here.
The biggest problem I have during the stage is when the fairies come out and start shooting that green shit at me. I can't stay on top of them fast enough, so I just resort to bombing through them. As long as I get back to full power before the boss, I don't care.
Sanae's first two spell cards are easy enough, but her third can be tricky. Sometimes you'll get lucky and the stray bullets won't get in your way too much, and other times they'll box you in.
I completely screw up Koishi's first spell card. I made mistakes and couldn't get into the right position to dodge those lasers. My mind gets mixed up on where I'm supposed to be.
Koishi's heart spell card is where I always eat shit and die. It's so annoying. I DO know how I'm supposed to dodge it, but I always have trouble getting started right. It sucks because you can hear that I almost made it through the spell card, but I barely got clipped by a heart at the end.

Seeing this makes me really, really want to open up extra stage, but mainly Orin keeps me from doing just that. Darn cat.

Yeah, the games are really fun, but they can be frustrating too.
For these kind of shumps (bullet hell), yeah, you need practice. A new player could do well on skill alone when starting out, but experience is needed for the harder levels and difficulties.
Extra stages are different from the regular stages, so I wouldn't say it's just about remembering patterns. To me, Touhou games require a combination of:
Skill, quick reflexes, bullet manipulation, and memorization.

You seem pretty good at it though. Do games like this require much practice?
(Judging from this vid, it's mostly about remembering patterns, or am I wrong?)
