Notes: A lot of people have asked me how you're graded when you're pulling out Bones/Glass out of people's wounds. Originally, when I was playing Second Opinion, I thought it was how close to the middle of the tray you placed the glass. In retrospect, I dunno where I got that idea from.
Anyways, it's definitely how close to the middle of the cut the glass stays when you pull it out. So basically, you want to pull it out directly perpendicular to the direction the cut is, and keep it in the middle of the cut as best you can.
About once per operation, save for special operations.

tip for people with trouble getting cools on glass, find the direction it needs to be pulled in, and then pull it out FAST. You'll get a cool almost everytime, if not all the time if you do it that way.

About the glass: my experience on Under the Knife suggests that it's how close the glass stays to the middle, as playing Life or Death a bunch of times without syringe ends up being only a B or A if I let the glass drag but easy S by keeping it steady. Definitely not the tray, though.
