Shelby is Clark's dog. The aim of the procedure that was going to be completed on Clark is to relieve the brain of pressure, but it results in either death or the patient staying in a vegetative state. The mental institution that Clark attended was a hallucination in his mind, as he was actually lying on his barn floor during his "mental period". Hope this helped.

@Ladydragontl shelby is Clarks dog...and the strange sound through out this episode was shelby barking at Clark's body in the barn,..so as you see this was all going on in his head, due to the fact the phantom took over him!!
