@azb24 the D at the end is thrown off a bit because the lead is playing a standard C behind it. (it's done with the baritone guitar in the song - I think). Right after the G in the chorus, using an open C chord and feathering the C note on the B string (off and on) will give you the "change" at the end of each measure ;)

I watched a bunch of videos of Aaron Lewis playing at his live acoustic shows to get the basic chord progression. Then I picked around by ear until I got that right.
Intro/Verse = A, C, G, D
Chorus = Am, C, G, D
Bridge = G, D, Am
There are some variations in there, but that's the basic structure.
