It probably needs a thorough cleaning of the engine, inside and out. I'll bet every gasket leaks, too. But, for now, all it needs is a valve adjustment and a quiet muffler.

The Volga is an incredibly reliable car. I owned one just like this in the early

1970's when I was working in Afghanistan.
