People laugh, but in the filming of the first series of Ultimate force, 74 UK asylum seekers were killed. That's not to mention the time when Ultimate force were filming on set in Pakistan and Ross Kemp accidently fired a rocket launcher into an orphanage, killing 12 children.

They could at least use a real grenade launcher. (I may be wrong here, I'm not an armourer.) You can see what definitely looks like a shotgun shell at 2:01. This may be some sort of grenade, but it looks too small to me.
