The second chord is not D.
It's kind of like a D11 chord without the 7th. Basically you take the first position C chord and you move everything up two frets. Also, finger the high E on the same fret you're fingering the A string.
so: x54031
but in this song the treble notes of that chord aren't really played. You could also play: 254xxx, if you don't mind lacking the high notes of the chord
He also plays this chord in Naked as we came, and sodom south georgia, and probably others.
