The original Hobson ran a livery stable in Cambridge. He hired out his horses in strict rotation, so that the customers would not wear out the best horses by overuse. "Hobson's choice" thus came to mean "take it or leave it."

As to your introductory rhetorical question, medicine in the US is a mess because of government involvement, particularly the tax preference for employment-based insurance.

"Hobson's Choice" did not mean "take it or leave it": It was a choice between a poor horse (a very unpleasant ride) or no horse at all (a very long walk). Thus, "Hobson's Choice" actually came to mean a toss-up between two equally unattractive options.
It's quite apt for my situation: a choice between staying in the place I love, near my family, but having no affordable access to health care -- or gaining access to health care but leaving the place and family I love: a real "Hobson's Choice."
