During this entire ordeal Blair and Chuck get in car accident, really horrific, which we later find out was meant for Nate, aside that Blair is the only one to wake up, Chuck is predicted to die. Devastated Blair goes to the hospital church and promises god of he saves Chuck she will marry Louis. Right after she promises a nurse tells her that he's woken up, clearly her first response was it worked, so to cut this short she leaves him to keep him 'alive' . Hope that helped (:
