First Tandem Skydive "We have a malfunction"

My first tandem skydive and the main tether and parachute don't open! All I hear is "We have a malfuntion".
He pulls the reserve at 10,000 feet. We free fell, flipping at over 160 mph.
My neck still hurts!

I make the reserve opening @about 7K'. You were falling @about 180MPH @the time. It's very odd that your TI couldn't deploy the drogue. He's the one who packed it. It only sits in a BOC. @180, I'll bet that reserve deployment was (AHEM) firm...

Looked like the drogue came off the bridle. The only drogue I lost was upon deployment. Bad opening, but the main eventually deployed.
