It was most definitely pilot error. He meant to throttle up (to gain altitude) and instead, accidentally moved the nozzle control lever forward, which directs thrust back (forward flight), instead of down (hover/vertical land). You can actually see that happening as the jet loses lift and starts to fly forward, with not nearly enough altitude to transition into forward flight. Google it.
