Opiates aren't associated with positive symptoms. Realistically, they can't cause them.

Then again, the writers have stretched the truth with diagnoses, cures, and treatments throughout the series.

omg I hated the ending! I was like YEEY it isn't a hallucination( cuz al the way to this episode I really was thinking that because it would be really like the writers) And then they just wrote it was still a halucination... I hate the writers, I know house can't get a happy ending yet.. but come on! Can't the man have some joy in his life?

I think I know which episode you're referring to (No More Mr. Nice Guy).

The thing is, the symptoms don't fit with neurosyphilis. They're more related to personality and memory, not necessarily hallucinations and delusions. House's memory is fine, and even though he's not as cold and distant as he was in the first season, he's still House.

After considering the contradictions, I'm pretty sure schizophrenia isn't the cause, but it's a feasible plot for Season 6.

We can only speculate...

You saw how in the prior episode he had only two choices left in order to diagnose his hallucinations; Vicodin, or Schizophrenia.

I believe schizophrenia is the cause, since it explains both hallucinations of Amber/Kutner AND the delusions of the detox/Cuddy sex. Vicodin doesn't cause those positive symptoms.

Can't wait until September, heh.
