It's not hard, I actually bought this game when it came out. The game includes step by step videos on how to do them. You can even watch them in the middle of the operation using that unidentified black box on the instrument stand. You may as well be Dr. Nick Riviera.

I agree with you. Its really not that hard to succeed in the surgeries. The main problem though are the hundereds of bugs, which make the game nearly unplayable (today)
