Yes, sometimes refusing cancer treatment is a rational choice (if the prognosis is really bad a person might want to spend their remaining months at home with family rather than in hospital). This case, howevere, was one of avoiding effective treatment with a good prognosis early on.

wrong. the third leading cause of death in america is cerebrovascular disease. the forth is chronic lower respiratory diseases (pneumonia, for example). nothing to do with drugs. world wide
the world wide classification of causes of death is not of simple classification. the best claim is to say: ischemic heart disease is the 4th leading cause of death, world wide - WHO.
