Launch of Columbia and STS-107. The orbiter was destroyed while attempting re-entry on 1 February 2003. All the crew perished.

There is a crew cabin video showing the astronauts during re-entry. I can upload it if you have'nt seen it already. It stops about 10 mins before the break up. There was a warning that showed the crew there may have been a problem..but the ground had way more information than the crew could see on board. If you want me to upload that vid then let me know.
