Every character in the song is based on a real person. Apparently, Robertson said he was inspired by the surrealist filmmaker Bunuel when he wrote the song. There's a great article discussing all the possible meanings of the song.

@skittlesareyum48 well Rick Danko used to tell folks "it's about whatever you want it to be, its about whatever it makes you feel." direct quote. he told me it was pretty much about a "visit" to heaven Robbie was quoted as sayin its about the impossability of sainthood. Levon Helm had as much to do with wriing this as Robbiw truthfully they all did. everyone mentioned in the song is about or relates to a real person that was in their lives at some point.
