The maxillary sinus natural ostium is hidden behind the uncinate process. However, for a safe and effective maxillary sinus surgery, its natural ostium must be seen to avoid a phenomenom called "recirculation".
There is no anatomical landmark for the maxillary sinus natural ostium and we propose the "m" line, which is a line from the most inferior aspect of the ethmoid bulla to the lateral wall. Wherever the line crosses the uncinate, the maxillary sinus natural ostium will be seen, always angled.
