Therefore, this looks like another Rove set-up.
bush gave a pronounced hand shake just before the throw. At this point the thrower knew to throw shortly after.

It was like a football teams play that always worked.

See, it was just enough to excite in the world more hatred. We love war don't we those who are still alive?

pay close attention to the handshake bush gave. it was too high and like a brotherhood shake. That was the cue to throw the shoe.
