This Kata came from one demostration in the Nihon Karate-do Kenkyu-kai, Does someone know the name of this form? Is one Hakusturu Kata?

this form is a crane based form...and there are many variations of Hakkustsuru and Hakkaku.

similiar movements to Crane Kata i've seen but the stances seem lower than most i've seen in other Hakutsuru kata

Yes Hakutsuru means White Crane :)
