They did this (coming on stage in dresses, as opposed to the '50s white-man variety show suits') to further underline the type of people that this song targets. The red-necks, homophobes, and bullies. Kurt was very aware of certain idiots and their intolerances to anything, or anyone different. Nirvana killed the big hair rock bans like Poison (and the egos behind them), and showed kids like us that it was ok to just be who you are "come as you are", when you like, how you like, I wont judge.
