READ THIS DESCRIPTION BEFORE ASKING ANY QUESTIONS.
The Mesmetron is a stun gun that can be found by speaking to Grouse at Paradise Falls. You are supposed to use it to stun certain people and enslave them via a slave collar that Grouse gives you. However, there are a few other things the Mesmetron can do, often for comedic effect and sometimes even to help you complete a quest (obviously other than "Strictly Business").
EDIT: You get no karma loss for stealing stuff as far as I know. Very strange as stealing so much as bent tins from owners shelves or bins nets you a karma loss. However, fitting them with a slave collar means a large karma loss, no matter how nice you are to them afterwards. You get no karma loss if people attack and kill a berserk person but if the head exploding effect takes place, that will count as killing them, and any karma gain/loss will take effect depending on who you kill. Simply mezzing someone will not cause a drop in karma.
EDIT: The green stick is the Repellant Stick that Moria gives you to complete a part of the wasteland survival guide. She didn't take it off me, but if she steals yours, use cheats if you desperately want it.
EDIT: Choose the second option with a slave collar in your inventory when you speak to a mezzed victim in order to fit them with one.
EDIT: The mesmetron should have an effect on your target. Make sure it hits for starters (the shot hits the target directly and doesn't skim it), save before you mezz and patch your game if still nothing happens. It makes no difference which part of the body you hit, just as long as you hit your target.
EDIT: An NPC stands for Non Playable Character and is self explanatory.
EDIT: You can get the Mesmetron by killing Grouse but you cannot complete the "Strictly Business" quest if you do so. The quest reward is only to gain entry into Paradise Falls, and so if you kill Grouse, you cannot get slave collars from him. If you lose or sell the mesmetron, you cannot find it at a second location. You must either find the one you lost, or get it again by a console command. It is the following within the brackets. EDIT. Ok, let me make this clear. Maybe the people going berserk isn't a glitch, but the fact that you get away with murder is!
EDIT: You can get more ammo from Grouse, although at an expensive price. You can also find it lying around, but these are few and far between.
EDIT: The berserk effect is open to discussion. I personally think it is a glitch, as there is a lot of evidence to support this. The manual states that "Assault: Attacking someone without provocation results in a major crime" which is when everyone becomes hostile to you. If the berserk effect occurs, nobody becomes hostile except the victim, who if killed, everyone else returns to their duties rather than punish you for causing it. However, the berserk thing does not occur all the time, as I have used it a few times and everyone turned hostile towards me without any other effects. Also, it could be that the programmers did not program it properly, saw this effect and thought it was so funny they decided to keep it in the game. Or it could be they actually programmed it to do this, going against their vision of a lifelike Fallout universe and the statements in the manual being a lie. So morons that enjoy disagreeing with me can eat this and maybe think about their argument in a bit more depth, as they aren't the game designers either.
EDIT: Save before you mezz someone and do not holster your weapon when you go up to speak to your victim. Each effect of the mesmetron has a percentage chance of happening. There is a list of NPC that can never be mezzed, and instead have an increased chance of going berserk. Mezz them from behind, definitely not when they're attacking you and don't use VATS for best results. That's all the advice I can offer about successful stuns with the mesmetron.
EDIT: Try mezzing unnamed characters. They are most likely to be mezzed and/or enslaved. Try using it while hidden and while the target is unarmed. Some named characters can be mezzed though. Even hostile characters such as raiders and talon mercs can be mezzed, although you will need to sneak up on them.
I know there are other videos on Youtube about this weapon but they don't include a minigun wielding villager wearing a skimpy night dress, do they? Enjoy!
Actually, I'd like to propose another edit, because often times I have mezzed raiders, TC mercs, and Enclave soldiers from the front in VATS while they were attacking, and they have, more often than not, been dazed and confused for me to enslave. Maybe it's just me getting lucky while I was making it difficult for myself.

@rockerhead247 For me personally, I have always mezzed them without VATS, because it saves ammo due to the chance of missing in VATS. It's not impossible to mezz someone while attacking, but it becomes very unlikely. You are indeed a lucky person. XD

Mesmetron power cells aren't expensive if you think about it.
Once you complete 'Strictly Business' you can enslave absolutely anyone and receive 250 caps for doing so. It costs 200 caps for 10 Mesmetron power cells. Using 10 Mesmetron power cells you can enslave 10 people. 10 x 250, you do the math. It's the best money making technique in the game, although it only lasts a little while because there is a limited amount of people in the game. (Excluding raiders)

@Widgetation I meant that it is expensive without doing money making schemes, losing karma and in comparison to other items and that you can't lower it's price in any way. I know that it is a great but evil way of making money.
