i have this game on xbox 360 and when i just get to the top of the water everything is
blank except the sound i shoot and i hear it shoot i swim and i hear it swim but i cant see her shoot or swim

there was another glitch in the game aswell. My dad was playing a level with trees and rain then there was this drawbridge that was meant to be lowered so you can get across and it wasnt. Turned out that the glitch was triggered by something in the previous level (the level where your riding the bike in the trees) went on the internet and it said if you turn left at a turning instead of right, the next level will glitch at the drawbridge which was what happened to my dad. what a crappy game. lol

I didn't like Tomb Raider game at all because I often stuck in one place without any clue...

Yeah ... i played one level (Mexico) from the beginning only because the automated shit in the end of the level didnt trigger.
Crystal Dynamics just suck.

It's generally quite a buggy game.
What would have caused this is something that you did (seemingly do every time you play) earlier in the level to cause this. That is how all the bugs happen in this game (something you did earlier on so a simple save and restart won't work).

@mattjasonhardwick im starting to think I was on treasure hunt and didn't realise you can't go on the ship because there are no treasures there.

I think I was lucky because I never had any serious problems on the PS3 version, the only annoyance I had was it sometimes forgot what treasures I had already picked up.
