The "american m16," as you say, which is very vague, since there are 4 variants of the M16 in use by US forces, has only been found to be unreliable in two situations. One, Vietnam, when ammo was loaded with an excess of dirty burning powder in an effort to increase the range(This was done by a dumbass senator who, like you, didn't know shit). Two, when it gets packed with sand. AKs fail from sand too, they're not precise.
