Das ist ein normaler Kurbelantrieb, der wegen eines Patentes Watt nicht zur Verfügung stand. Das kleine Zahnrad ist nur Ziierde.

Smaller the number of parts the better the tolerances & sturdier/more efficient. This converts reciprocating motion into rotary and changes the gearing ratio at the same time.
This ^ was the first commercially successful engine.

Look at how many times the flywheel turns compared to revolutions of the arm. By varying the number of teeth on a cog, different advantages could be had. Huh... he almost coulda made a transmission :)
