Love this video and very helpful, but once I have embedded this into web page or blog, not sure how to get this to allow each participant to type name in so that it shows up on spreadsheet???

Thanks for the feedback. If you make your first question a text question and then label the Question Title 'name,' the first question the students see will ask them to fill in their name.
