It was done by changing the way Quake renders the completed view. Six views of the in-game environment are produced for every frame: forward, back, left, right, above and below.
Then a lookup table is used to source each pixel required for the resulting fisheye view from the six views that Quake rendered using its normal rendering routines.
By far the most fiddly part of the whole modification was getting all the pitch, yaw and rolls to translate properly.
