Alone, Jack reflects on his past with Kate. The memory of her lingers still in his heart and mind, though she is not with him.
Finally he confronts her- Perhaps to make the memory not a memory anymore, but something present; something real.
He is rejected. Though he wiped her tears before, his hand does not lay on her face now; when he had held her hand, she now pushes away, and drives off into the night, leaving Jack, very much, alone.

tbh Sawyer and Kate is just a fling. Always has been and always will. Sawyer may love Kate and Kate may feel for Sawyer but she doesn't love him in return. She's always hankering for Jack's attention which is why the second time she slept with Sawyer was out of jealousy for Jacket. First time was out of pity. Don't sound like love to me.

Same with Jacket. Jack only kissed Juliet out of comfort not out of affection.
