As seen in this video the patient takes part in a mini time out upon entering the OR The patient is asked name, DOB, procedure and side and verifies such. What is not well known is that the patient may have no recollection of the event as Versed is often given prior to taking the patient to the OR and this caused amnesia. This is an important part of the process and the patient should remember their involvement. I gave that last consent and don't remember it, What if I'd changed my mind?
