i think kurt's stomach problems were fucked up, but they had managed to fix it i think by the time he killed himself. he had a pinched nerve in his spine from his scoliosis, and this combine with his irritable bowel syndrome left him with a lot of stomach problems, which he uses as a total cop-out for being a heroin addict.
