@lijikafuk I'll assume that you don't know the concept of Death Note. If you did, this would make more sense.

Basicly the notebook is a Death Note, a notebook where if your write the name of a person in it, they die. Sasuke in this story found and picked up a Death Note and met the Shinigami attached to it, Naruto. The Shinigami has to stick by him. Since Naruto extended Sasuke's life when he was supposed to die, Naruto died instead, because that's against the rules.

This needs to have an English translation!! This is by far my favorite SasuNaru doujinshi and it makes me cry almost every time I watch it. I love the songs used in this video, I love Death Note, and the thought of Sasuke being Light and Naruto being Ryuk just fits perfectly in my opinion! I LOVE IT!!! T^T i wish it were in English
