Just for clarification, only 1 seatbelt should & can be used to safely/correctly install a carseat. Using 2 or 3 seatbelts at the same time increases the risk of injury by possibly puting pressure on the carseat where it is not intended to withstand pressure.
5 point harnesses without the shield are safer, too. They spread the crash forced out over a broader area of the body without risk of "Shield Slap".

Hi there. Just wanted to let you know that this car seat appears to be expired. It needs to be replaced immediately. Also, it's apparent that it's installed extremely incorrectly.
In an accident, this child could be killed or severely injured. Please encourage to his parents to see a CPST immediately and NOT to drive with this child until the seat has been replaced and CORRECLTY installed.
