I had the same problem, it got worst every month. This malfunction comes from 'the MacBook MagSafe DC-In Board'. Depending of the model, you'll have to replace it. I have the first Macbook Pro model, that piece is integrated in the whole I/O left board(with one USB and audio I/O), so it's an expensive part. I've replaced it myself since my laptop is 4 years old and out of warranty. If your warranty it's active, USE IT AS SOON AS POSSIBLE. If not, I'll do my best to help you.Tambien en español
Yeah. It's like having the power brick and the cord with prongs on it in one. Then if you want to extend the thing, you take out the prongs and insert the cord where the prongs were. It's a really good design.
This video is a fail... you basically showed us what Apple did a hundred times.

It goes from a cable to a little WHITE box with the apple logo on it, the to the wall =)

either way! It can plug directly from the brick or an extension cord if needed!
