More likely because the film is being weaved through the gate, thus you get slight gate weave. That's my guess.

The black spots are dust on the film, the lines (white or black) are scratches, and I don't know
how the film gets shaky.

Nice, another one of those awfully boring films, at least this one has a interesting subject. I wonder where it came from, it seems to be kinda mistreated (black spots, lines and shaky picture). Also this machine seems similar to the "RCA Motion Picture Projector" in the film from 1957... Kodak often rebadged projectors for sale under their name, like the Elmo 16-CL which was sold as Kodak Pageant something something...

It gets shaky because the lab was not very careful when making the copy, like making the machine run much faster than recommended as to get the job done quick. It's possible, but the picture gets shaky. Or it could be that the perforations are a bit torn, but that's improbable.
