@emsdream2011, actually morphine has been taken out of the CHF protocols for AHA. It shows that patients in CHF who were given morphine had a predictably poor clinical outcome.

@ron6081 Thanks for the comment! Keep in mind we are referring to our county protocol which the county's medical director writes - it may thus differ from ECC guidelines. Incidentally, although some studies show an association of increased need for mechanical ventilation/risk of death, AHA ECC Guidelines 2010 does not provide guidance on the use of morphine in the setting of acute pulmonary edema, nor has the AHA actually made a formal position removing its use in ACC/AHA joint statements.

Emsdream: what are your state/local protocols for CPAP?

@ffmhc62 CPAP is an ALS level skill. It is primarily indicated for Adult Acute Cardiogenic Pulmonary Edema, along with standard medication therapy (i.e. nitroglycerin, morphine and furosemide...the latter being appropriate only if the patient is already on diuretics.) The only approved CPAP device in the County is the Boussignac CPAP at this time (helps the on scene FF medics restock with the ambulance crews.)

@freshhh723 Hi there, thanks for the comment. The 25 lpm is actually for our county's required CPAP device. When set to 25 lpm it provides up to 10 cm H20 of pressure. The actual device is called the Boussignac CPAP system if you want to google it.
