I think that sometime after the 19th season, I saw someone else's named called and they were in the bathroom. When he said they lost a contestant, I thought he meant they died.

When Bob was in crutches, he was attacked by a Samoan. I think the contestant had to pay an insurance fee to fix Bob's leg. Today, you cannot attack a host. You can hug, you can hi-five, but you cannot pick up or attack a host. That will give the host a huge medical bill.
