The difference between the Hertz in each ear determines the frequency of the beat and there's nothing placebo about it! you need headphones or a wide stereo field for it to work.
So 300Hz in one ear and 305Hz in the other will combine in the brain and induce a beat or 5Hz which is the difference between the two signals.
