Fieldhome is located in a beautiful park-like campus setting on 100 acres on the border of Cortlandt Manor and Yorktown Heights in New York. For our short-term sub-acute rehabilitation residents, we have a 50-bed dedicated wing and a spacious 1,500 square foot state-of-the-art rehab treatment center with a separate occupational therapy suite. We provide therapy 7 days a week for patients who need it and can tolerate it.

From your very first hour at Fieldhome, our team of professionals are dedicated to meet your needs and to make certain that you and your family are comfortable.

Our private rehab rooms have telephones, cable tv, DVDs, internet access and we can provide a laptop for you. We offer flexible personal-choice dining options and even a private dining room, where you can enjoy catered meals with your family and friends.

Our goal is simple, to return our patients to their homes and normal function quickly and safely.

Fieldhome is a person-centered care facility recognized for its outstanding quality of services. Offering a continuum of care, including skilled nursing, hospice and respite, Fieldhome is home to the Seabury, our on-site Assisted Living and Memory Support Residence. We also have the Early Learning Center which offers intergenerational programs and activities, and brings a very special element to our total campus.

We are conveniently located near major highways, metro north trains and only 20 minutes from White Plains.
