on windows i used to import my signature in jpg in order to sign pdf's with the free PDFXchangeViewer !!
i think with your second solution it will word great !!
-with the first solution the image will be to a separate page and it will be not possible to add it below the "sign now" !!! (sorry for my bad English )

To add an image into a PDF, using Preview and OS X 10.5, you will have to create a new PDF with the image in it then merge that PDF page into your existing PDF.
The other option is to copy the entire contents of your PDF and paste it into another application, then modify the contents, then print another copy of the updated PDF.
If there is another method, I cannot think of it at the moment.
