To enable third person shoulder mode you just open up console using ( ~ )Tilde key and type Thirdpersonshoulder then press enter. Try it out! It takes a bit of tweaking to get third person working flawlessly.
If the console doesn't open up for you then you need to enable the option. Go to options then Keyboard/Mouse then make sure Allow Developer Console is set to Enabled.
