He's not alone. He has friends and family. And its not his responsibility. This takes place in the 80s, before most people knew how you could get aids. People didn't know any better because it was a relatively unknown disease at the time. As he says in the movie, "We didn't know what aids was, or how you could get it." So it isn't his fault at all. Its just bad luck that he happened to get it.

Truly the BEST scene in the film, but upon watching Denzel's character enter into his own metamorphosis, especially the follow up scene,(not shown here) where Washington can't get the aria out of his head after he leaves and goes home, is AMAZING. He tucks his "baby Clarisse" in her crib and then suit on and all, lies next to his wife in bed who is asleep.. and just holds her..so near and dear to his heart. It is here where we see the depth of Washington's character change.

"You must live, I am life itself! Your heaven is in my eyes! You are not alone. I shall collect all your tears I will walk with you and support you! Smile and hope! I am Love! Are you surrounded by blood and mire? I am Divine! I am Oblivion! I am the God who saves the World I descend from Heaven and make this Earth A heaven! Ah! I am love, love, love." And the angel approaches with a kiss, and he kisses death - A dying body is my body. So take it. I am already dead matter!
