Aha! Well, now I get it!!! :-) the "watch in high quality"-option was already here for a while... :-) maybe because we have a Dutch version of Youtube? xx

You're right, adding &fmt=18 does not increase the resolutin especially the sound anymore but now I noticed just under the Views: "watch in high quality"...

That's how the quality in resolution and sound would have been. Now viewers just need to click "watch in high quality".

Looks like Google is automatically adding &fmt=18 to high resolution (sound and video) uploads. :^)
