If I mug a pensioner and she dies of a heart attack, brought on by the stress of my assault, am I responsible for her death? Absolutely.
He was attacked from behind without warning, of course caused his heart attack.This was an act of aggression. If he was in their way they could easily have moved him using non-violent means. What's to stop them picking him up and carrying him away? Then they use their crimes to slander protesters, claiming that they impeded his treatment. Utterly disgraceful.

Copper jabbed him in the back (0.21) and Mr Tomlinson didn't react. Coppers don't like that, they LOVE to see fear and a reaction when the abuse someone. Therefore, they set upon him with greater force and pushed the poor guy over. What A bunch of thugs.
