He was never inside the casket, really... the whole "sacrifice your friend" thing was symbolic. Or at least, that's what we were meant to think, though it's conceivable that Pious magicked the effigy to do to the Custodian whatever we did to it.
I still think the screaming is the effigy, not the Custodian. I dare not believe otherwise, for fear my mind would not survive it...
