Just my opinion, I think this song is about an open casket funeral. "The Doctor tells me he's dead, I don't believe him. I look at him in the coffin (cold and catatonic), dead. But i don't believe it, so I yell at him to wake up. And to stop playing dead, because I don't believe it." He's the perfect enemy because the worst thing that could happen to the man who doesn't believe it, is the one in the coffin truly being dead. That's why it's passive aggressive. That's why he's the perfect enemy.
