3 things you should do:
1. Remember to hook in
2. Do a hang check to verify you hooked in.
3. Right before launching, pull up on the down-tubes to verify that the harness is pulling on you. This is a second hang check you should do out of habit. It verifies that you did a hang check.
You would have to forget to do ALL 3 of those things in order to launch unhooked. You didn't do ANY of those things.
