This patient had a large silver filling with a pin. Extensive decay underneath the filling would lead someone to consider extracting it. The patient was able to avoid needing a root canal at this time but knows this will most likely happen. A crown should be placed on the tooth but the patient can only afford a large filling at this time.

  • The xray shows the tooth that is 3D in a 2D format. The pin was not in the pulp chamber and came out easily

  • Vitrabond by GC was placed in the deepest areas. Ketac Nano for a larger base followed by Filtek Supreme Ultra for the outer surface. This patient does not have the money for an implant. She realizes that this is a heroic attempt. I did not use a matrix band. I chose to use cord to retract the gum tissue and treat the preparation like a class five restoration. No sensitivity so far.

  • The burr are being used with an electric handpiece which can be as high as 40,000 rpm. i usually set it for under 20,000.

  • you should be proud. i wish all dentists, in fact all doctors were so conscientious and took their oath so seriously.

  • The primary Xray shows that the pin from the amalgam is in the pulp chamber, did it have any affect on the final restoration ?

  • It's a heroic attempt to save that molar with poor prognosis! What kind of liners/composite did you use? Did you use a matrix band? Any symptoms after?
