It isn't 3D, and it isn't "one of the first" of anything. Both isometric fake "3D" graphics (what this game uses) and true 3D date back to the 1970s (though they didn't see much use in video games until the early 1980s).

That's how the game actually sounds. It's the Genesis version, so you have low-quality cartridge samples being played by a soundchip designed in the '80s.
