72 yr old male had cataract extraction elsewhere in 2009. Surgery was complicated and IOL was damaged. Patient was unable to manage with photophobia and maddox rod effect and sought removal and replacement of defective IOL. Dr Gimbel demonstrates his IOL removal technique and the use of the remnant capsular membrane to capture the optic of the replaced lens for secure central fixation of the new IOL.
