The nasal cavity is divided into two halves by a partition made of cartilage and bone, called the nasal septum. The two halves are the nostrils. In as many as 80% of all people, the nasal cavity is not divided equally. When this occurs, it is called a deviated septum. For most people, this imperfection does not cause any problems, but for some, it can lead to difficulty in breathing and even chronic sinus infections.

Symptoms vary from person to person, depending upon the severity of the deviation. Some only experience symptoms when they have a cold or other respiratory infection, and symptoms are usually worse on one side of the nose than the other. Symptoms include:
1. Blockage of one or both nostrils
2. Nasal congestion
3. Frequent nosebleeds
4. Frequent or chronic sinus infections
5. Sinus pain

The only way to correct a deviated septum is through surgery. A septoplasty is usually performed on an out-patient basis, with either general or local anesthesia, and usually takes 1-1 1/2 hours. This procedure is performed entirely through the nostrils, and if it is the only procedure performed, it does not cause any bruising or swelling following surgery. A septoplasty can also be performed in conjunction with rhinoplasty.

OPERATIVE TECHNIQUE:
The septum was infiltrated with 3.0 cc of 1% lidocaine with 1:100,000 Epinephrine. Pledgets soaked in pontocaine and 1:1,000 Epinephrine were placed in the nasal cavity. A left hemi-transfixion incision was made and the mucoperichondrium was elevated on the left side of the incision and extended to beyond the junction of the bony & cartilaginous septum and the mucoperiosteum was elevated. The septum was disarticulated from the bony septum and the contralateral mucoperiosteal flap was then elevated off the bony septum. Takahashi forceps were then used to remove the deviated bony septum. For the spur, a 4 mm osteotome was used to remove it. A strip of cartilage along the floor up to the anterior nasal spine was removed allowing a swinging door septum to be created. The septum was midline after this maneuver. No opposing bilateral perforations were identified. Septal cartilage was harvested for grafting for the rhinoplasty leaving approximately 10 mm of septal support at the anterior and caudal septum. The incision was then closed with a running 4-0 chromic.

  • Hello I have surgery for a deviated septum about 10 days ago and i am doing ok but can you please tell me if it is healing properly as the left nostril seem to have heal but the right nostril is still sore to touch but there is like a red spot or lump near the septum at back which isn't really painful but was wondering if it infected or whether it will be better in the next few weeks don't have a follow up appointment to early january. how long will the stuffness last as it still there thanks

    It would be best to have an appointment with your surgeon, as he/she knows the extent of your surgery and exactly what was performed. Congestion should improve within two weeks. It sometimes takes longer depending on the patient. Thank you, and I hope this helps.

  • Good Afternoon, I was just told by my ENT i have a deviated septum, Which i will be having surgery to fix. What is the cause? Is it something you are born with or Does a injury to the face cause it? I had major face damage due to a tornado when i was 9, Now 44. Wondering if it just got worse as time went by... Thank Tou for your time. PS Wishing i lived in Beverly Hills i would love for you to be doing my surgery.

  • I had septoplasty done exactly 2 months ago. I have alot of stuffynuss which comes n goes. Deviated side is alot better but the other side get blocked at times but overall i am happy. I have a problem with my eyes tho..they constantly feel tired and tight and swollen and have huge bags. PLEASE tell me this goes away?

  • I have deviated septum but I think its genetic because two of my uncles also have this issue. My question is: could excessive bleeding in the brain occur if septoplasty is performed, if God forbid it does, what are the consequences?

  • is it possible that it is a mid vault collapse, the problem goes away when i flare my nostrils and pull up on the nose, another way that i get relief (and the problem is gone) is to again flair my nostrils and then using my left index finger push TOWARD the indentation on my nose which makes the left lateral wall of the nose sit on top of the septum more, does this sound like something you have come across, if so whats the probable cause and wut type of procedure should i go for , ty 4 ur time

  • hello dr nassif, quik question, i am having a hard time figuring out wuts wrong with my nose , i have been able to eliminate the sinuses or nasal polyps as the cause of my congestion(surgery has fixed that) also have been to doctors who have looked INTERNALLY with a camera (which i viewed as well) and have said "they dont see wuts wrong" the problem is i feel a contact point on the left side of the nose , at the same spot where i have an indentation that looks like a mid vault collapse

  • @TheMutedVid its anesthesia , aka sleep gas and no way in hell would any body want to be awake during that
