Everybody that says that it's a "mistake" or "graphical mess-up" is wrong. It's a scar.

During Halo 1 there was a Covenant ship call the Infinite Succor. The Flood got onto it, he was the Elite that led a force to go rescue a minor Prophet from it and then destroy it. One of the Elites under his command was infected and he had to fight him, that's how he lost part of his face. It's one of the stories in the Halo Graphic Novel.
